Fall-Risk Reduction & Resident Safety

Fall-Risk Reduction and Safer Daily Living for Seniors

At Magnolia Senior Care, resident safety is an important part of daily care. Our caregivers assist with mobility, transfers, and daily activities according to each resident's individual needs. RN-guided care planning, awareness of changes in mobility, and communication with healthcare professionals help us identify concerns and support appropriate safety measures.

How it works

What families can expect.

Mobility support

Help moving safely

Reminders to use walkers and canes, stand-by or hands-on help when walking, and assisted transfers, including mechanical lift transfers when the care plan calls for them.

Environment

Awareness of the home setting

Caregivers pay attention to everyday hazards such as clutter, lighting, footwear, and bathroom safety, and to times of day when a resident is more likely to need help.

Care planning

Individual safety needs in the plan

Each resident's care plan describes the mobility help they need. RN-guided care planning helps the team decide when the plan should change.

Changes

Noticing what is different

New dizziness, weakness, confusion, medication changes, or a near-miss can all raise fall risk. Caregivers report changes so they can be reviewed.

Communication

Families and providers informed

Magnolia communicates with families and the resident's healthcare professionals about falls, concerns, and changes in mobility.

RN-Led Clinical Excellence + Family-Centered Care

The role of RN oversight

Sosena Mekuria, RN, helps the team look at the whole picture behind fall risk, including medications, hydration, sleep, and changes in strength or memory, and decide when a resident's provider should be contacted. Registered Nurse visits are scheduled according to resident needs and each home's care arrangements. Trained caregivers provide 24-hour resident support, and Magnolia does not provide 24-hour on-site nursing.

Good to know

Limits and fit

No home can prevent every fall, and Magnolia does not promise specific safety outcomes. The goal is to reduce risk where possible, respond appropriately, and keep families informed.

Questions families ask

Frequently asked questions

FAQ

Can Magnolia guarantee my parent won't fall?

No home can guarantee that. Magnolia works to reduce risk through mobility support, care planning, and observation, and keeps families informed.

FAQ

Are caregivers there at night?

Both homes have 24-hour caregiver staffing. Awake overnight care may be available for an additional fee if a resident's needs call for it.

FAQ

What happens after a fall?

Caregivers respond, check for injury, seek medical help when needed, and inform the family. The care plan is reviewed for anything that should change.

Services are provided when appropriate for the individual resident, based on assessment, the negotiated care plan, and each home's ability to meet needs safely. This page is general information, not medical advice.
